The following forms are available from the Division of Boiler Safety for download: 

• Application for Exam

The Application for Examination form is required for all new inspectors requesting commissions in the State of Delaware. Applications for Examination must be notarized and accompanied by payment with a copy of the inspector's National Board working card and commission certificate. Once this information is received by Boiler Safety the inspector is required to call (302) 744-2736 to schedule a test date. • Boiler Installation Form

The Boiler Installation Registration form is required on any new installation in the State of Delaware. This form is completed by the installing contractor and submitted to Boiler Safety as soon as possible to schedule a boiler inspection with an authorized inspector. NOTE: It is the responsibility of the contractor to submit a Contractor's Operational Report OR a CSD-1 Operational Report prior to obtaining an operating certificate.

• Report of Inspection. This form is submitted to Boiler Safety by an authorized inspector within 30 days of conducting an inspection. Download form
